## Our Story

Dr. Joshua Reischer, a **board-certified internal medicine physician**, spent years on the front lines of care, watching hours every day disappear into collecting, documenting, and organizing patient information instead of caring for patients. He didn’t see this as a documentation problem. He saw it as an **access problem**. Patients struggled to prepare. Staff struggled to keep up. Providers were buried in fragmented data before the visit even began.

At YC Startup School, Josh met Aaron Rau, a serial entrepreneur who had built software platforms used by organizations like Goldman Sachs and Medicare. Together, they shared a simple conviction: healthcare needed **intelligent digital teammates**, not more tools.

That belief became Health Note. Today, Health Note powers millions of patient interactions nationwide, deploying AI-driven **Access and Intake Assistants** that help patients prepare earlier, guide them to the right care, and ensure clinicians walk into visits with clarity instead of chaos.
